You can access Home Care Services by going through Home Care Agencies’ websites or by contacting them directly. Patients are guaranteed the right to choose the Home Care Provider of their own choice. Hospitals and other long-term facilities are required to provide a list of all the Home Care agencies to patients who need Home Care at the time discharge from their facilities, including any hospital sponsored or owned Home Care Agencies.

Patients, family members, physicians, case managers, hospital/nursing home discharge planners, social workers, friends, and neighbors may help or make recommendations regarding Home Health Services.

Home Care is a wide range of Health Related Services provided in the privacy of people’s own homes for the purpose of maintaining and restoring health and well-being.
